1. All Contract Caregivers must complete an Availability Calendar for each month.
  2. Each Availability Calendar contains a date in the upper left hand portion of the form that indicates the date by which the form must be returned to the Heartland office.  These dates are established by determining the length of time it will take to complete the schedules for all patients, and allows time for them to be completed before the beginning of the new month.
  3. Print your name as well as providing your signature on the two appropriate lines.
  4. Each caregiver must indicate the days that you are “Available” to work, or “Not Available”, for each day of the month.  Do not leave the days blank and expect the scheduler to understand that you were available to work.
  5. Do not complete the calendar based on the schedule you normally work for your present client/patient.  There are no guarantees that a caregiver will only be scheduled in a certain client/patients home.
  6. As you complete the calendar, remember that you must make yourself available at least two (2) weekends per month. “Weekend” is defined as 7:AM Saturday to 8:PM Sunday.
  7. Contractors who fail to properly complete and return the calendar by the “Due Date” may be removed from the schedule for the following month
