As required by the law governing Nurse Registries, Heartland Registry must maintain personnel records on all contractors. These records must provided proof of certifications or license for a number of items that are maintained in the file. These licenses or certifications must be current, up-to-date, and a copy of the renewal placed in the contractors folder. The following list indicates those items that must be in the contractors folder, with proof of renewals as needed.

Valid, up to date, Florida drivers license.
Florida RN/LPN/CNA license/certificate.
Home Health Aides are required to have an approved course by the Florida Department of Education
Social Security Card.
Current, up to date CPR card.
L-2 background screening (Fingerprinting) Conducted on all caregivers contracted after June, 2010, and on all previously contracted caregivers, 5 years from the date of the last screening.
Statement of Good Health on all newly contracted caregivers, and renewed in the event of a 90 day lapse in service.
Proof of Florida Automobile Insurance coverage, in the contractors name.
Proof of Professional Liability Insurance, with a minimum coverage of $1,000,000.00/$6,000,000.00.
Proof of training in “Assisting with self administration of medication”. (CNA/HHA’s only)
Proof of one 4 hour course in HIV education.
Proof of HIPPA training.
Proof of Bloodborne Pathogens training
Proof of Cultural Diversity Training.
